• We accept cash, credit/debit cards, CareCredit, HSA/FSA and most insurance plans.

• Co-pays, deductibles, and out-of-pocket costs are due at the time of service.

• Payment plans are not available for any service.

• We accept a variety of insurance plans; please check with our office before your visit.

• Coverage verification is recommended before services are rendered.

• Patients without insurance may be responsible for full payment.

• Patient confidentiality is strictly maintained.

All prescription refill requests MUST be submitted through the secure Patient Portal.

For safety and documentation, we cannot process requests submitted via phone or email. Please allow up to 2 business days for your provider to review and approve the request.
